How to refer:

Email: your referral letter and Mental Health Treatment Plan to hello@wellnessavenue.com.au. You can address the referral to a named practitioner below, or to Wellness Avenue and we’ll match your patient with the most suitable practitioner.

What to include:

• Patient’s name, date of birth and address

• Presenting symptoms or diagnosis

• Current medications

• Number of sessions referred for

• Whether a Mental Health Treatment Plan has been prepared

 

What happens next:

• We contact your patient within 24–48 hours of receiving the referral

• We send you a letter confirming referral acceptance

• At the session 6 review, we send a progress letter to support your review and any re-referral

 

Fees + availability:

Standard session (50 min) $210.00 | Medicare rebate $101.55 | Out of pocket $108.45.

The initial consultation (up to 90 min) is $250. Full fee payable on the day. After-hours and weekend sessions available, all via telehealth.

 

Medicare (Better Access): Patients with a Mental Health Treatment Plan from their usual GP, or a GP at their MyMedicare-enrolled practice, may be eligible for Medicare rebates on up to 10 individual sessions per calendar year: an initial course of up to 6 sessions, with further sessions following GP review.

 

Who we see: Women aged 16 and over. Medicare (Better Access), private and WorkCover clients, and some NDIS participants (please contact us to discuss).
